Instructions

1. Cut light blue cardstock to 5 1/2" x 5 1/2". 
2. Cut black cardstock to 4 1/8" x 4 1/8".
3. Cut white cardstock to 4 1/2" x 4 1/2".
4. Print information on white cardstock and adhere to black cardstock.
5. Adhere black cardstock to light blue cardstock.
6. Add self-adhesive embellishment as shown.
